New patients will need to fill out our intake paperwork before therapy can begin. To make the process as smooth and efficient as possible, we provide links right here on our website.
Find out which forms you need to submit, select the appropriate link and submit the completed documents to our practice via e-mail. Filling them out in advance saves time.
Intake Forms - These forms will need to be completed prior to your appointment. You have the option to fill out the form online using one of the links below or can come into the office and do it in advance. You can also come in an hour prior to your scheduled appointment and complete it then.
Please bring your ID and insurance cards to your first appointment so we can make a copy for billing purposes.
Records Requests - Records can be requested by accessing the link below. Please understand that there is a fee for records and only a summary of care will be provided. Complete records can be sent to a new provider for continuity of care free of charge. Please allow 7 days for all records requests.
